Patient Rights & Responsibilities
At InterCare, we believe that good health care involves a partnership between you and our health care team. This partnership is based on a set of patient rights and responsibilities.
InterCare's patients have a right to:
- Care regardless of race, religion, nationality, sex, age, disability, marital status, citizenship, or ability to pay.
- Privacy.
- Confidentiality of Information.
- Be treated with respect and dignity.
- Information about your health conditions.
- Help make decisions about your care: say no to treatment; and be told what could happen if you refuse treatment.
- Talk about concerns and suggestions to a supervisor.
- An explanation of your bill and financial aid.
- Know the name of your bill and financial aid.
- Know the name of the person providing your care.
- A copy of your medical record according to our policy.
- Know how we assess and treat pain.
- Communication assistance.
It is the responsibility of InterCare's patients to:
- Provide a complete and correct medical history.
- Provide correct and current insurance information.
- Tell the Doctor or Nurse if you do not understand your illness or instructions.
- Follow the instructions and advice given by the Doctor and Nurse.
- Tell the Doctor or Nurse about changes in your condition or problems with your treatment.
- Be considerate of the rights of other patients and of InterCare staff and property.
- Call us if you will not be able to keep your appointment.
