InterCare is dedicated to improving individual health and reducing health issues in the communities we serve. We pursue our vision through the delivery of a broad range of primary care services and promoting open access for all.

InterCare's main focus is on disease prevention, wellness, and regular and timely treatment of chronic conditions.

We pride ourselves on excellent communications with our patients and on a strong health education focus that helps our patients and their families become involved in their own health care.

Why Use Community Health Centers?

Community Health Centers focus on trusting patient relationships with a regular provider and a team-based approach to delivering care with emphasis on treating the whole person.

Quality and safety are central elements of the care delivered at InterCare along with good communication and information sharing.

InterCare has also recently moved to increase access to care through expanded hours of operation, creation of a centralized patient call center, and the addition of more support services such as behavioral health care, transportation, and case management for at-risk children.

In addition to medical and dental care, InterCare offers a full range of comprehensive support, education, nutrition, case management, and transportation services.
